[Animation image shows the spinning circle fragmenting and disappearing from the screen and then text appears: Health Care Homes, Patient-centred, Coordinated, Flexible] With so much on your plate, our health system can seem hard to navigate and disjointed. That’s where Health Care Homes comes in. [Camera pans down to show an animation image of a male walking up to a General Practice building] It’s a way of caring for people with chronic conditions which puts the patient first. [Camera zooms in and the animation image shows the doors to the building opening and the male entering the reception area and standing beside a male, and two other females] A health care home is a general practice or Aboriginal Community controlled health service with one team co-ordinating your care. [Colour of animation image changes from coloured to black and white] It could be the practice you currently visit but with an emphasis on co-ordination to help you get the care and health services you need. [Animation image changes to show the patient standing between a male and female outside a building] Your health care home can be flexible in the way they look after you. [The camera zooms in on a male dressed in green on the left side and the animation image shows him holding up an iPad and text appears: Shared Care Plan, Health Goals] If you want to talk to someone in your care team you won’t always have to have an appointment with your GP. [Image moves to the right and the patient appears on the left of the screen using a Smart phone while the man in green also uses a Smart phone] You might call or message the nurse, or they might call you to see how you’re going. You are the focus of your care team and your care team will listen to your needs and preferences.
